Physical Therapist - Digital Health Specialist - Hybrid Role (2 days remote/3 days in clinic)

Must be loced in Sebring, FL (or within driving distance). This is a hybrid position working 3 days in person our CORA Sebring clinic locion and two days working remotely as a Digital Health Specialist. The Digital Health Specialist (DHS) provides digital health services, including virtual care and remote therapeutic monitoring. The DHS participes in the development of the digital health program and serves as a liaison between pients and in-clinic treing providers. This is a full-time, hybrid position (40 hours/week, 5 days/week). Must also be willing to obtain PT licensure in other stes.

Wh You’ll Need:

· Diploma from a CAPTE-approved Physical Therapy Program

· Physical Therapy ste license in Florida (or in process)

· Dedicion to providing exceptional quality of care to each of your pients

· Minimum 2 years of relevant clinical experience (outpient)

· Experience providing digital health services preferred

Wh You’ll Do:

· Make a powerful impact on your local community through inclusive physical therapy trement

· Develop and deliver a personalized plan of care for your pients -- a diverse pient populion with both orthopedic and neurologic diagnoses

· Objectively measure pient outcomes using cutting-edge software

· Efficiently document evaluions, trements, re-evaluions, and discharge notes

· Uses digital plform to deliver telehealth visits including evaluion, assessment, and trement plan development.

· Performs Telehealth and Remote Therapeutic Monitoring (RTM) services including interactive communicion and monitoring.

· Works closely and supports operions to deliver digital services and meet pient care and staffing needs.

· Communices with the primary treing clinician providing revisions in trement as approprie.

· Serves as a liaison between providers and Director of Digital Health to optimize and grow remote services CORA.

· Maintains regular reporting and da analysis of regional service metrics.

· Participes in program development and marketing efforts as directed by the Director of Digital Health.
